Field documentation is written for people who enter records on a phone in poor light. It sets out what a daily report must contain to count as evidence, how a defect is linked to the report and the photograph behind it, which approval a request for information waits for, how measurements are recorded, and what happens to a service request after handover. A defect carries no photo field of its own.
